How this screener works

Short answer

ADHDFalcon presents the official six-question ASRS v1.1 adult self-report screener electronically. Items, response choices and order are unchanged. The official patient form uses shaded boxes: four or more answers in those boxes is a positive screen. This rule produces a screening outcome, never a diagnosis or a clinical assessment.

Instrument origin and permission

The six-question screener is drawn from Part A of the Adult ADHD Self-Report Scale v1.1 Symptom Checklist. Its origin and research publications are linked below. The licensor permits electronic versions with attribution and does not permit other modifications. This site does not reproduce Part B, the full 18-item checklist or other screening instruments.

The required attribution and copyright notice appear below, on the screener, review step, free result and report. Naming the instrument and citing sources does not imply affiliation, endorsement or clinical review.

The shaded-box rule

Items 1 through 3 have shaded boxes for Sometimes, Often and Very Often. Items 4 through 6 have shaded boxes for Often and Very Often. Four or more answers in shaded boxes is a positive screen. Fewer than four is below the screening threshold. There are no weights, additional scales or extra points.

Electronic presentation

The website uses radio buttons, one question per screen, back navigation and a progress count. Official instructions are shown verbatim. A separate note explains using radio buttons instead of marking a paper box. Before submission you review all six answers in the official grid with five response columns. Shaded cells include an accessible text label.

The free result repeats the grid and includes limitations. Buying a report never changes the rule or the result. Optional context after purchase only orders reflection worksheets. AI has no role in calculating the outcome or writing the deterministic report.

Storage and deletion

Answers, the outcome and optional context are stored together in an encrypted vault. Unpaid retention defaults to seven days, and paid personal-data retention defaults to 365 days. A reduced payment record can remain for accounting after personal data is removed. The live screener and offer show their configured retention periods.

Use Delete my answers and report on the result or report page to remove personal data and revoke private links. No answer or screening outcome is sent to analytics, payment metadata, share pages or email text. The assistant receives outcome and shaded count only when you choose the per-message checkbox.

The 6-question Adult Self-Report Scale-Version1.1 (ASRS-V1.1) Screener is a subset of the 18-question Adult ADHD Self-Report Scale-Version1.1 (Adult ASRSV1.1) Symptom Checklist.
